Since 1981, we have developed a simple plan for ensuring proper care of your pets while minimizing your cost. There are no tests, no examinations, no laborious forms, no long list of deductibles and exclusions. Our pet care plan is simple, inexpensive and can provide you the comfort of knowing you can care for your pets when they need you.

How Your Pet Care Plan Works

Your premium is paid only once a year. The amount is based upon the age of your pet. When your pet requires care, the normal costs are totaled. You pay only the amount less your scheduled discount.

How to Enroll

To enroll a pet in the Pet Health Care Plan, the owner must complete the Enrollment Form available below:

Pet owner must also provide the Plan with the appropriate premium for each pet enrolled. Coverage will begin seven (7) days after our hospital receives your enrollment application. Enrollment premiums are not refundable for any condition or reason.

Payment of Services

Accounts must be paid in full at the time of the service or discharge, a 50% deposit is required prior to hospitalization. If proper payment cannot be made at the time of service, eligibility will not be valid. Premiums are not refundable.

Exclusions

• No emergency services after normal business hours
• Veterinary or emergency care performed by non-staff members and facilities

• Giardia vaccines
• Current illness or injury
• Not transferable to another pet
• Cosmetic surgery
